Dear Oley Friends,
As an enteral consumer or caregiver and member of the Oley Foundation you have an opportunity to share your tube feeding experiences in an effort to impact the development and improvement of enteral products/materials.
An internationally respected product manufacturer, who wishes to remain anonymous, is coordinating an on‑line survey and is particularly interested in your experiences with enteral feeding. Your answers will provide them with much valued "end‑user" information as they move forward with plans to meet the needs of "your" community.
Along with the reward of knowing that you're making a difference, the product manufacturer is compensating Oley $5,000 for coordinating this effort and is pleased to award $50 to each of the first 100 adult consumers or caregivers of adult consumers and the first 100 parent/caregivers of pediatric consumers and pediatric consumers (14 or older please) who complete the survey.
The survey will take approximately 30 minutes to complete. It's worth mentioning that if you are unable for any reason to complete this survey in one sitting, you will not be able to log back in at a later time to finish. You will need to start over. Your responses will be kept in the strictest of confidence. All results are analyzed at an aggregate level and not individually.

The survey will remain open until 100 adult consumers/caregivers and 100 pediatric consumers/caregivers complete the survey. If the research company has collected the responses it needs from your particular population, you will see this message: "Thank you for your interest in this study, but we have met our quotas at this time and the survey is now closed."
